A guide to checking care Help desk

Build a question list that fits your visit

Mark the questions you plan to take to the visit. Make a short list you can use, leaving room to ask for a clearer answer.

The question to open with

Start with what you cannot yet explain in your own words. Choose the questions that would help you fill that gap.

Tick the questions below. Use “Print selected questions” to get a paper list, or “Copy selected questions” to paste them into your notes.

When saving is available, your ticks stay in the app you use to open this website, such as Chrome or Safari, on this device. The message below the list tells you whether they can be saved after you close the page. This checklist sends no answers to a service.

Which answers do I still need before deciding on the next step?

Select a question to put it on your take-along list.

Choose the questions you want to ask

The person and the assessment
The ingredient and its evidence
The bill and the next step

Choose the questions you want to bring.

Keep an unanswered question open, even if the rest of the discussion feels clear.

Bring the list into the conversation

Ask for a short answer first, then the record that supports it. Request a plain explanation when a term is getting in the way.

For each answer, note what is clear and what remains open. The paper sheet has room for both.
