A guide to checking care Visit desk

Prepare for an assessment, with room for questions

Bring the concern you want assessed. Leave room to discuss other ways to address it and why a proposed plan fits.

The question to open with

Write a brief note naming your concern, when it started and the activities you want to resume. List medicines and supplements for the clinician to review.

Bring any records you want discussed. Ask the service what it needs before the visit, and who will read it.

What do we need to understand about my concern before discussing a treatment?

Start with the problem. Leave the treatment decision open.

Prepare a note for each part of the visit

Your concern

Name the change you have noticed and the part of daily life it affects. Ask what still needs to be checked.

Tests or an examination

Under medical board guidance, tests or an assessment in person must be arranged when the care standard calls for them.

For every proposed test, ask which question it addresses, who will read the result and how that result might affect the plan.

A choice of plan

The state medical boards’ national group issued its guidance in May 2026. It says selecting a listed treatment without a proper assessment falls short of proper care.

Ask what other options could address your concern, including waiting for more information.

Each step needs a purpose you can explain after the visit.

Finish with a written next step

Ask the clinician to sum up what was found, what is still uncertain and what happens next. Include who you can reach with a later question.

If a word gets in the way, ask for it in plain language. A short note you understand is more useful than a long note you cannot use.
